Freshman Health Requirements for the 2013-2014 school year
Please remember that all incoming Freshman physicals must be returned to the School Nurse by July 1, 2013. All Freshman students must show proof of one dose of Tdap, regardless of the interval since the last DTaP, DT or Td on your student’s immunization record prior to entrance into Glenbard West High School. *Students who have not provided a current physical examination including all immunization requirements for entrance into 9th grade will not be able to register and attend classes in August 2013.
New Changes in Immunization Requirements for Sophomore,Junior & Seniors for the 2013-2014 school year
All Sophomore, Junior and Senior students will be required to show proof of one dose of Tdap (combined tetanus, diphtheria, acellular pertussis) vaccine regardless of the interval since the last DTaP, DT or Td dose. Most students may have already received the vaccine and simply need to provide verifying documentation from the family health care provider. See pertussis link below.
General Information
Glenbard West Health Services is staffed by two certified school nurses and one certified nurse assistant / secretary. Students are assisted with immediate health concerns and chronic medical challenges. The school nurses work closely with all members of the pupil personnel team, which includes guidance counselors, school psychologists, deans, and social workers. The school nurses identify and plan for students with health concerns with a goal of optimal health. Toward that goal, chronic illness, hospitalizations, attendance, and injuries are monitored.
The health center is not a “resting” station, nor is it a treatment center for anything more than minor first aid. No medication, including over-the-counter (Tylenol, Advil), etc.) is dispensed unless a medication form is completed and signed by parent and physician. The medication forms and medication are only kept for the current school year and must be renewed yearly. Per school policy, students in possession of any "drug" will be referred to the Dean's office for disciplinary action. Exceptions to this include those students who carry doctor prescribed asthma inhalers and epipens. School Authorization Medication Forms ( see below) for all medication must be completed by both the doctor and parent and on file in the nurse's office.
A student who becomes ill during the school day should obtain a pass from his/her teacher before going to the health center, unless an emergency situation exists. If a student needs to contact a parent, they should report directly to the Nurses' Office. Students must adhere to the school policy regarding the use of cell phones. Students are not allowed to text message or call a parent via a personal cell phone during the school day to arrange dismissal. Students who need to be excused from school due to illness must report to the health office with a pass. Students will not be sent home unless a parent/guardian or parent designee has been contacted. Any school accident must be immediately reported to the teacher in charge of the activity so a written report of the incident can be made.
Parents may request in writing a temporary excuse from physical education in writing for up to three (3) days in any one semester. If any more than three days are required, a physician must write the excuse. The student should report to the Health Center with any written excuse before their first hour class. The Health Center opens at 7 AM. P.E. teachers, coaches and/or trainers will be informed of the excuse. Students out of gym on a P.E. medical release for a week or more will be required to complete a weekly written sport folio assignment. If a physical adaptation to P.E. is required, please have doctor complete Physical Education Medical Release Form (see below).
If students are ill, they should not attend school until they are well. Injuries sustained at home should be treated at home or in a physician’s office. Parents must call Attendance (630) 942-7480 before 10:30 AM on each day of absence to give the reason for absence. Failure to do so will result in a truant absence.
When Should Your Child Stay Home?
• Bad Coughs/Cold Symptoms which interfere with your child’s ability to keep up with school activities, a constant runny nose or continual coughing.
• A skin rash or red eye from an undetermined cause. Your child may return to school with a doctor’s note stating your child is cleared to attend school.
• Any illness that is accompanied by decreased energy and an inability to sustain usual daily activity level.
• Diarrhea or Vomiting until illness is over and your child is symptom free for at least 24 hours – without use of medicine.
• Fever of 100° or > by mouth (fever of 99° or > under the arm). Your child should be fever-free for 24 hours without the use of medicine before returning to school.
• Severe, persistent pain, including injury.
• If antibiotics are prescribed for communicable diseases, they must be given for 24 hours before the student may return to school.
Please help us control the spread of illness by reminding your child to use tissues as needed and to wash their hands frequently. Thank you for your help in this matter. |
